ITinvolve has a viral organic approach to knowledge management where knowledge can be associated with objects.

Stakeholders can ‘follow’ objects, crowd source knowledge and collaborate in real time.

In this screenshot we see that only information that is pertinent to the issue at hand, Microsoft Exchange, is available.  

This is a good overall approach to try and manage "real life" ways of working in IT.
